We are currently seeking compassionate, Maryland-licensed Nurse Practitioners who want to make a meaningful impact by providing in-home health assessments in the Laurel area and surrounding communities.

The Opportunity
This is a contract-based, per diem role to start—perfect for Nurse Practitioners seeking flexibility, independence, and purpose-driven work. As our client base grows, there is strong potential for this position to evolve into a consistent part-time opportunity for the right fit.

In this role, you’ll conduct non-invasive, in-home health assessments that allow you to build meaningful connections with patients in a comfortable, familiar environment. These visits do not involve prescribing or treatment—instead, the focus is on reviewing overall health, identifying gaps in care, completing medication reconciliation, and offering supportive health education.

As we expand, we’re looking for Nurse Practitioners who share our passion for health equity, dignity in care, and bringing quality healthcare directly to the community.

Key Responsibilities:

• Conduct in-home visits and perform physical health assessments.
• Review members' medical histories and current health concerns.
• Complete medication reconciliation and identify potential gaps in care.
• Educate members on preventative care, chronic condition management, and available resources.
• Document findings accurately in our secure health records system.
• Communicate any notable concerns or care needs with the DiscoverU Health clinical team.

Minimum Qualifications:

• Master’s Degree in Nursing (MSN) required.
• Active Nurse Practitioner (NP) license in Maryland (or multi-state if applicable).
• Current RN license and board certification as a Nurse Practitioner.
• Current CPR certification.
• Minimum of 1 year of experience in clinical practice, primary care, or home health preferred.

Preferred Skills & Attributes:

• Strong communication and interpersonal skills.
• Confidence in working independently and making sound clinical observations.
• Experience working with diverse and underserved populations is a plus.
• Comfortable with electronic medical recordkeeping and mobile devices.
• Adaptability and a high level of professionalism in dynamic or home-based environments.
• Culturally sensitive and committed to promoting equitable access to care.
