Clinical Support Liaison

Full Time | Day Shift | 10am-6:30pm

$18-$22/hour

(Pay based on experience)

As a Clinical Support Liaison, you will carry out our mission to improve the experience of aging together by providing support to the medical and nursing staff by ensuring orderly and uniform access to resources, manuals, instructions, etc. across nursing units. Responds to family and resident questions to ensure issues are addressed timely and accurately. Answers phone calls and call lights on units, relaying information to the appropriate party.

What will I do as a Clinical Support Liaison?

  • Act as a liaison among the interdisciplinary team, as a central source for directing inquiries, coordinating responses, and ensuring outstanding items have been addressed timely.
  • Demonstrate excellent customer service and communication skills with residents, families, medical and clinical personnel.
  • Seek to provide timely answers to questions and/or directs the inquiry to appropriate personnel. Manage discharge survey process.
  • Coordinate and arrange transportation for short-term patients throughout facility.
  • Ensure patient and family notification, appropriate payer source, appropriate mode of transport, and provide necessary documents to accompany patient to appointment.
  • Support nursing team to review standard non-clinical processes; i.e; resident names by assigned room, armbands, etc. May be asked to complete an all-resident/room review of various non-clinical situations/environmental rounds.
  • Perform clerical duties including typing, filing, scanning, copying, answering phones and call lights to ensure timely responses to inquiries.
  • Maintain nurses' station in a neat and orderly manner. Arrange reference materials, manuals, tracking notebooks in consistent location across nursing units.
  • Ensure necessary copies of various records are available for timely access.
  • Participate in quality assurance audits.
  • Perform other duties as assigned, including filling in for resident appointments, transportation, on-site clinics.

What is required in this position?

  • Requires High School diploma or equivalent and some clerical education; experience may be substituted for clerical education.
  • Prefer prior experience in long-term care or health care environment.
  • Basic computer and typing/data entry skills; Microsoft Office applications; general office skills including filing, organizing, scheduling and tracking.
